REQUIRED: Last name must be at least two characters.

Phone Number

REQUIRED: Phone number must contain at least ten digits including area code.

Email Address (Optional)

INVALID: Email address is optional, but must be valid if included.

Order Receipt Number

REQUIRED: Order Receipt Number must be a five-digit number.

Total Price

REQUIRED: Valid Total Price is required.

Product Description

REQUIRED: Product Description must be at least four characters.
